The Leprosy sections have the secondary goal of addressing the loss of skills and knowledge by health care providers to suspect and diagnose leprosy owing to the reduction in number of cases seen over time.\r\n\r\n", "description": "Kenya has a large TB disease burden and is ranked 15th among the 22 high burden countries that collectively\r\ncontribute about 80% of the world\u2019s TB cases. The TB case notification rate (CNR) rose from 51 to 338 per\r\n100,000 population between 1987 and 2007 and is currently on a slow decline. As in the rest of Sub-Saharan\r\nAfrica, the large increase of TB is attributed primarily to the Human Immunodeficiency Virus (HIV).\r\n\r\nOther factors that may be contributing to this include the high poverty levels with consequent socio-economic\r\ndeprivation. This is evident in urban areas where there has been a phenomenal growth of slums and slum\r\npopulation. The increasingly large urban slum population has led to an increase in the proportion of TB cases\r\nnotified to the DLTLD from urban areas. For example in 2005, over 35% of all notified TB cases in Kenya were\r\nfrom the five largest urban areas of Nairobi, Mombasa, Kisumu, Nakuru and Eldoret, reinforcing the known fact\r\nthat poverty and TB are closely related. The implication of this observation is that a general improvement in\r\nsocioeconomic conditions may be the answer to TB control in the long term. However, case finding and specific\r\nchemotherapy are the only methods that are known to have an important and immediate impact on the transmission\r\nof TB. The finding of TB cases and the provision of efficacious chemotherapy is the mainstay of TB control\r\nactivities of DLTLD.\r\n\r\nThe DLTLD adopted the Directly Observed Therapy Short Course (DOTS) strategy for the control of TB in\r\n1993 and achieved countrywide geographic DOTS coverage in 1997. The country achieved the 1993 World\r\nHealth Assembly global TB control targets of 70% detection of infectious cases and cure 85% of the detected\r\ncases by 2005. Further, the country has adopted the TB control Millennium Development Goals of halving\r\nand beginning to reverse the mortality and prevalence of TB by 2015.\r\n\r\nThe DLTLD, in line with international trends, has launched several new approaches to increase access to\r\nDOTS and truly expand population DOTS coverage. These approaches include community based TB care\r\nDOTS (CB-TBC), Public-Private Mix for DOTS (PPM DOTS), collaboration between TB and HIV control\r\nprograms and the development of an elaborate advocacy, communication and social mobilization strategy\r\naimed at influencing communities to seek care early when TB symptoms occur and to remain on treatment\r\nuntil this is completed when treatment is initiated.\r\n\r\nThe Division of Leprosy, Tuberculosis and Lung Disease (DLTLD), previously the National Leprosy and\r\nTuberculosis Programme (NLTP), was launched by the Government of Kenya (GoK) in 1980, combining the\r\nhitherto Kenya Tuberculosis Programme which existed since 1956 and several leprosy control projects which were\r\nin existence in Western, Coast and Eastern provinces since the early seventies. The DLTLD is mandated to develop\r\npolicies and guidelines, mobilize political support and resources and carry out activities aimed at controlling both\r\nTB and Leprosy so as to eventually remove the threat to public health that these diseases currently pose.\r\nCurrently the DLTLD is receiving support from the Government of Kenya, through the Ministry of Public\r\nHealth and Sanitation and bilateral and multilateral donors and technical agencies such as the Presidential\r\nEmergency Plan for AIDS Relief (PEPFAR) through the Centre for Disease Control & Prevention (CDC) and\r\nUSAID, the WHO, the Global Fund to fight AIDS, Tuberculosis and Malaria (GFATM) and the Canadian\r\nInternational Development Agency (CIDA) through the Royal Netherlands Tuberculosis Association (KNCV)\r\n", "guidelineType": "CLINICAL", "datePublished": "2015-02-26"}
